This conference will set the stage for your practice’s successful submission for medical home recognition. More importantly, it will create the foundational understanding of what it takes to transform your practice to the model of patient-centered care. We encourage practices to bring key members of their leadership and clinical teams so they can champion the effort within the practice.  There is free parking on-site and a continental breakfast buffet will be served. 

Please have each individual attendee from your practice register separately so that we can obtain an accurate head count for the event.
